The electrocardiogram in Fig. 1 shows a sinus rhythm of approximately 60 beats/min with two widely separated Pwave components. This reflects significant atrial conduction delay. The first three beats in Fig. 2 are similar to Fig. 1 with two distinct components, as shown by an asterisk and arrow in Fig. 3. A 25-year-old man with idopathic atrial flutter and high grade atrioventricular (AV) block is described. Postcardioversion, sino-atrial (SA) and AV nodal dysrhythmias occurred. Treadmill exercise during atrial flutter increased AV conduction to 2:1, and normalized SA and AV nodal function following cardioversion. A neurogenic basis for these arrhythmias is hypothesized.
